How was moving from Children’s Services to Adult Social Care?
Wakefield Council are running a project looking at people’s experiences moving from Children’s services into Adult Social Care.
They have developed a survey and want to hear from young people, families, carers, and professionals about their experiences.
They would also like to speak to any groups, or have 1-2-1 conversations with, young people, parents or carers who have experience of this.
